The mini has been created using bits from Devotee Malicant (legs and torso), Preacher Josef (head) and the eviscerator from the chain weapon sprue. The backpack used to be Covenants backpack once... Obviously putting a half-eaten apple on top of your head must be somewhat a symbol of religious devotion in the 41st Millennium.
