What causes broken images and is there a fix?

Normally I just think "hmm, shame" and move on with my life, but the images I'm looking for at the moment, well I really want to see them, and a whole slew come up as broken. I'm not really fussed about knowing the cause (could be interesting info, could be boring code rubbish) but I'd love to hear that the images can be fixed, and that "broken image reported" isn't a message designed to mollify viewer or a signal to admin to delete image or something.

OK Feel the love then.
We get reported images back in the awaiting approval listing. Quite often I see images reported as broken if they've been hosted elsewhere (photobucket) or if they haven't loaded properly, internet feed stealing gremlins or whatever. These get a pat on the head and sent back out into the playground.
Sometimes if they are getting repeatedly broken we can "Grab" the offsite images and store them in coolminiornot's database, this usually (95% of the time) sorts things out.
Occasionally things go pearshaped and the we try to contact the owner and get them to check and re-submit.
