With a Multi-level Mall and under ground car park, a sewer under the town, the full works....

One issue I could see with 3D cars is that the KickStart game-play video showed the cop car with four circles on it, which I'm assuming are 'seats' for the survivors to be placed on - how would you represent the same thing?

Well assuming you don't have too many cars on the table, you could simply remove a character when they enter the 3d car on the board, and place them on the corresponding car token just off the board edge in the seats.
