GD "title tags" on display bases

Hi!
I'm currently working on my GD entry and have a nice display base for it. I'm looking to add a "title tag"(for lack of a better word for it) to like, like for example on this entry:

Is it a printed sticker on plastic card? Laser engraved on metal? What kind of shop can produce this? Any help is greatly appreciated!

Sproket did a guide on making them here https://sproketsmallworld.blogspot.co.uk/2016/09/making-titlename-plaque-for-display.html I know quite a few people have started using this technique but you need access to a laser jet printer for it to work

Great tutorial. Thanks for the link. I have been trying to find an engraver in the US and it has proven to be very difficult to get one of a kind plates done.
