Question about reward items

So the campaign book says in some quests you immediately reward a hero upon picking up a quest token. Several questions ensues
1. Does a hero have to discard one of his equipment if there is no space?
2. Can the player choose another character to slot the reward to upon receiving?
2. If this hero gets killed by another player, does he have to give up the quest token thus relinquishing the reward to his killer?

1. If the hero has no card slots available and he wants to keep the new card, he'll have to discard something.
2. Yes.
3. Usually the scenarios tell you to exchange the Quest token for the reward card. So once the reward is collected, the Quest is no longer in the game. Either way, cards can never be stolen.
