Tough, but how Tough ?

According to the skill Tough
Tough: The Survivor ignores the first attack he receives every Zombies' turn.
So, how about, you open a door to a room, and a Runner gets an extra activation, moves one and lands on a square with two Actors, one is a Tough guy. Does he ignore the attack, as it is not the Zombies' turn ? My gut feel is to go with the skill as in the spirit in which IMHO it was intended, and say 'yes' he ignores it. The next problem is that the Runner now in their square acts on the Zombies' turn. It is about to do two wounds, does Tough kick in again ? My gut feel says 'no'. Reading the rules as written though, the wounds occur the other way around. However there is a possibility that the Actor ignores the first attack in both phases ?

I've played it as a "once per round" ignoring of a wound. So if the survivor with tough is hit by a car, or randomly shot, or bitten by an extra activation, he gets to ignore that one hit, but that's it until the zombies have had an entire turn of activations and the players begin again (basically regaining his toughness when the noise is removed from the board)

I'm sure some would disagree on the player-created damage, but I think it's fair enough. As for extra activations, I think you're right, it's how the "spirit" of the game seems to trend.

The wording states 'every zombies turn' I don't see how it's possible to suffer any damage from non-zombie attacks in the zombie turn.
However as a 'rules-as-intended' I have worked under the assumption that Tough ignores zombie attacks made in the player turn by extra activations, but not damage from players shooting at or driving over the Tough character (haven't played a car scenario yet, only Scenario01 7 times, just can't beat it).
